Katie is our new care co-ordinator at Yaxley

Our new care coordinator, Katie Frankum, says her role is about building connections with patients, organisations and social care to ensure people get the right help when they need it.

She mainly deals with concerns about mobility, frailty, support at home or the process around moving into care or residential homes. She works with GPs,  patients, their families, social care workers and charities, such as Age UK, to find the best pathways. 

She has a degree and Masters in psychology with a special interest in mental health issues, anxiety and depression. Katie said: ‘One day you can be helping someone with social services referrals and the next thing could be supporting someone with anxiety. You need so much knowledge on local resources because you deal with such a range of issues. 

‘Generally we have an initial telephone call to see what the patient would like, or, if they have mobility issues we visit for a home assessment and go from there. The best part is helping people, making that difference and being that person where, if they have an issue, they can ring someone up and get a response. We try to be really on the ball and it is those connections you build that make the difference.’
