New Patient Registration

To be eligible to register at the practice, you must live within the practice catchment area.

We will accept new registrations if you reside in the following areas

How to Register

It is easy to register!

Just complete our online GMS1 Electronic Form and a Questionnaire for Adults or Under 12s Form.

Physical copies of the form can be picked up from surgery reception. You can also ring us on 01733 240 478 with your details and we can post them to you.

You will also be required to provide a proof of address and identity.

All our new patients are offered either a New Patient Medical or an NHS Health Check, depending on eligibility.

Temporary Services

If you are ill while away from home or if you are not registered with a doctor but need to see one, you can receive emergency treatment from the local GP Practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local Practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that Practice.

To register as a temporary patient simply contact the practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a Practice in the town or area where you are already registered.

Your Named GP

All patients registered with this practice are required to have a named GP who is in charge of their care. This does not affect which doctor you see on a day to day basis. If you have a strong preference for which GP you are registered with please let us know and we will try to accommodate your request. If you need to know which GP you have been registered with, please contact us.
