It is easy to register!

Just complete our online GMS1 Electronic Form and a Questionnaire for Adults Form or Under 12s Form.

Physical copies of the form can be picked up from surgery reception. You can also ring us on 01733 240 478 with your details and we can post them to you.

You will also be required to provide a proof of address and identity.

All our new patients are offered either a New Patient Medical or an NHS Healh Check, depending on eligibility.

Disabled Patient Facilities

We have reserved car parking spaces for the disabled near the staff entrance. The patients can ring a bell and enter via staff entrance, which is located just a few metres away from the disabled parking spaces. Wheelchair access to the building is both from the front entrance and carpark door.

We have several wheelchairs available for our patients to use. They are located in Reception and near the staff entrance. Our Reception team members are available to help patients with special needs.

All consulting rooms are on the ground floor, and therefore wheelchair accessible.

The health centre operates a hearing loop system.
