Carers information stand at Yaxley
Primary Care Network staff in South Peterborough manned an information stand for carers at Yaxley GP practice to promote Carers Week.
Health and wellbeing coach, Ellie Martin (pictured right) and social prescriber Carole Gill talked to patients about the local and national support available to those who are carers.
PCN staff also shared information at other local surgeries including at New Queen Street in Whittlesey.
Dawn Farey, who leads the Personalised Care Team for South Peterborough PCN said: ‘The stands were very successful we were able to identify new carers that were not registered the practices and provide them with carers packs we had made up. We also met with patients that needed support with their health and wellbeing, which created new referrals for the Personalised Care Team and Pharmacy Team.’
